It's the kind of rash that it's time to see a Dr for!
If he's had it that long, and it's bothering him, and the cream you have isn't helping, time to get something else.
I think it's safe to say it's not chicken pox. I don't think it's hives, but our hives come in solid huge patches when dd gets them and they move around....but hey, they *could* be.
I would get him checked for sure, especially since you're starting to get concerned!
I wouldn't rush him to emerg by any means, but see your Dr or a local clinic.

I am with the Chick: time to get it checked out.
It is hard to tell from the photos; it sort of looks like Sara's eczema but not exactly. It could be an allergy but I am surprised it would last that long unless it was something he was coming in contact with a lot (and then I would think it would spread). It may just be heat rash, but even that doesn't usually stick around for long.
Whatever it is, the poor little guy is uncomfortable if he is scratching and anything that hangs around for a while should be checked.

I'm pretty sure it's not eczema. I've had eczema pretty much all my life and it doesn't look like that. But everyone's eczema is also very different so I might be wrong.
I'll call the dr's office when they open at 9am.

No, he has not been close to a deck.
I can't think of anything that would cause that. Nothing has changed in food or laudry detergent or diapers etc. No fever either.
It's strange.
My dr's office is amazing, I know that when I call, they'll fit me in today.

The last two summers I was getting a similar looking rash (hard to see up close in photo) on my forearms. I had it narrowed to resting my arms on our patio chairs. It got really bad after sitting there and reading for a long time on the weekends(before I figured it out).
Is he sitting on a plastic chair of some sort? Because that is where the contact would be, kwim?

If you aren't sure, I'd take him to the dr but not in a panic if you get what I mean. The last few weeks have been really hot where we are. It could just be the heat or that his legs are coming into contact with something that they weren't when it was cooler and he had his legs covered with pants. If it's gone away and then come back, I'd suspect something that he's coming in contact with.

Hi Gen, are the bits fluid filled? If that is the case, I get that under my wedding ring. It is a chemical reaction rash mixed with trapped sweat. My thought is, he might be getting it from his diaper - perhaps a combination of heat and sweating with the chemicals from the diaper.
I would go to the doctor for sure, but, if it does wind up being what I said, you might want to try a different kind of diaper - or even having him go without for a bit - if that is possible.
